INSECTS, RODENTS, & ANIMALS NOT PRESENT - Comments: OBSERVED LIVE ROACH ACTIVITY IN MULITPLE AREAS IN THE FRONT FOOD SERVICE AREA APPROX 5-10 LIVE ROACHES UNDER THE SHELVING AREA WHERE THE ESPRESSO MACHINE IS LOCATED UPPER REAR RIGHT CORNER BEHIND THE LOWER REACH IN COOLING UNIT AND INSIDE OF THE WALL COVERING ALONG THE SIDE OF THE WALL OF THE SWING DOOR WHERE EMPLOYEES ENTER, MUST TREAT AND REPAIR. APPROX 8-10 LIVE ROACHES BETWEEN THE HAND SINK AND THE COFFEE STATION ON THE REAR WALL ACROSS FROM THE MAIN COFFEE SERVING LINE. OBSERVED 3 SMALL ROACHES CRAWLING ON THE FLOOR OF THE SAME AREA AND ANOTHER 2 LIVE ROACHES CRAWLING IN THE REAR FOOD PREP AREA ON COUNTER NEXT TO THE LARGE DOUBLE DOOR OVEN. INSTRUCTED TO CONTACT PEST CONTROL AND HAVE FULL SERVICE WITH SERVICE LOG SHEET AT RETURN INSPECTION. PRIORTIY VIOLATION 7-38-020(A) CITATION ISSUED.
