NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OVER 15 LIVE SMALL FLIES WERE FOUND ON AND UNDER THE SODA FOUNTAIN CABINET AND DIRTY DRAIN BENEATH. OVER 25 MOUSE DROPPINGS WERE FOUND IN THE CORNERS OF THE REAR STORE ROOM AND TWO DEAD MICE WERE FOUND IN A NEARBY CATCHALL TRAP, THREE HOLES WERE FOUND IN THE NORTH OUTER WALL BRICK WORK, REPAIR THE WALL, CLEAN UP ALL EVIDENCE OF PEST'S AND CHECK THE BUILDING PERIMETER FOR ADDITIONAL HOLES AND HARBORAGE. HAVE THE PEST CONTROL PROVIDER PUT A COPY OF THEIR CURRENT BUSINESS LICENSE IN THE LOG BOOK.
