PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: 6-501.12: OBSERVED A BUILD UP OF DIRT, DUST, GREASE, FOOD SPLATTER AND FOOD DEBRIS ON THE FLOORS UNDER AND AROUND ALL COOKING AND NON COOKING EQUIPMENT AND ALONG THE LOWER WALLS. ALSO NOTED AREA JUST AT THE BAR ENTRANCE WHERE THE TEA IS PREPARED A BUILD UP OF TEA DEBRIS AND STANDING WATER. INSTRUCTED TO CLEAN AND MAINTAIN ALL AREAS AS A MEANS OF PREVENTING POSSIBLE PEST AND OR INSECT HARBORING.
