INSTRUCTED TO REMOVE ALL PRODUCT FROM THIS UNIT, CLEAN ALL OF THE BUILD-UP AND DEBRIS FROM THE ENCLOSED SECTION BELOW THE DOOR, TO THOROUGHLY INSPECT ALL COMPARTMENTS OF THE COOLER TO ASSURE NO LIVE ROACHES ARE IN THE UNIT AND REQUEST SERVICE VISIT FROM PEST CONTROL COMPANY. ALSO OBSERVED MICE DROPPINGS ON PREMISES, 10 ON SHELVES IN OFFICE AREA AND 5 ON SHELF IN REAR ON BACK WALL, MUST CLEAN AND SANITIZE ALL AFFECTED AREAS. SERIOUS VIOLATION 7-38-020
