NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED 2 LIVE ROACHES OF VARIOUS STAGES ON THE COUNTER INSIDE OF THE OFFICE, APPROXIMATELY 40 LIVE FRUIT FLIES IN THE REAR OF THE STORE IN DRY STORAGE ALONG THE WALL AND ON BOXES AND 15-20 MOUSE DROPPINGS ON THE FLOOR INSIDE OF THE WALK IN FREEZER. ALSO OBSERVED APPROXIMATELY A 1 INCH IN DIAMETER HOLE IN THE BACK DOOR FRAME. NO PEST CONTROL LOG AVAILABLE DURING THE INSPECTION. INSTRUCTED TO CLEAN AND SANITIZE THE NOTED AREAS AND PROVIDE ADDITIONAL PEST CONTROL TO ELIMINATE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020
