NEAR ENTRANCE OF FOOD SERVICE KITCHEN IN THE KITCHEN PUMP ROOM OBSERVED APPROX 20-30 DROPPINGS, IN THE KITCHEN BEHIND THE STOVE NEAR THE UTILITY SINK APPROX 15-20 DROPPINGS ON FLOOR. OBSERVED APPROX 20-25 SMALL FLIES ON THE CEILING NEAR THE ENTRANCE OF THE KITCHEN PREP AREA, OBSERVED APPROX ANOTHER 10-15 SMALL FLIES IN THE REAR DISH WASHING AREA UNDER THE COUNTER AND ROUND THE DISH WASHING MACHINE. INSTRUCTED TO CONTACT PEST CONTROL AND HAVE A SERVICE BEFORE THE RETURN INSPECTION DATE GIVEN. WITH PROOF OF SERVICE SHEET FROM PEST SERVICE PROVIDER. PRIORITY FOUNDATION VIOLATION 7-38-020(A) CITATION ISSUED.
