PREVIOUS SERIOUS VIOLATION CORRECTED, 7-42-090 - Comments: Observed the following previous minor violation # 18 not corrected from report # 2166507, dated 4/23/18: #18 Found approximately 140 mouse droppings in the following areas: on the floor in corners of the cafeteria office; on the floor along the wall behind the dish drying rack in the kitchen; on the floor behind the piano in the lunch room; on the floor in the closet and in the restroom of pre-k room 110; in student cubbies and on the floor of the large storage closet and in the smaller closet of room pre-k room 103; in the student cubbies and on the floor of the teacher's closet in pre-k room 102; on the floor of storage closet and teacher's closet in room 101; lower interior of refrigerator (not in use) on the floors of teacher's lounge (3rd floor); and on floor in small closet, on floor next to washer/dryer and on floor next to stove in parent's room 112. Must remove all droppings, clean and sanitize areas. Must eliminate pest issue. Serious violati 7-38-020. Pest activity noted on site during inspection in parents room, pre-k restroom, and pre-k classrooms 101, 102, 103, and 110. Pest control service receipts not provided by facility of any recent service since last inspection 4/23/18. Instructed must contact pest control for service, remove all droppings, clean and sanitize affected areas. Must eliminate pest issue. Critical violation 7-42-090.
