ADEQUATE HANDWASHING SINKS PROPERLY SUPPLIED AND ACCESSIBLE - Comments: 5-202.12 (B-D)--Observed two (2) metered faucets at hand sinks in boys bathroom on 2nd floor running for less than 15 seconds. Repair or replace to ensure continuous flow of water for 15 seconds is provided.

Blocked or unsupplied sinks force employees to skip handwashing, spreading fecal bacteria and viruses to food.

Handwashing sinks must be accessible, properly supplied, and used only for handwashing.

Each sink needs: Warm water (100°F minimum); Soap in dispensers; Paper towels or air dryers; Waste receptacles; Signage; Clear access; Used only for handwashing.

INSECTS, RODENTS, & ANIMALS NOT PRESENT - Comments: 6-202.15-- Observed exterior doors of main entrance to 001 and 002 have gaps at base in center. Provides access to exterior. Instructed to pest proof and seal exterior doors from pests, and to maintain.

Pests spread numerous diseases through contamination.

No pests or animals allowed in facility.

No evidence of pests; No live animals except service animals; Effective pest control program.

PLUMBING INSTALLED; PROPER BACKFLOW DEVICES - Comments: 5-205.15--Observed three (3) loose faucets in the following bathrooms: Girls bathroom near 130/131. Main building, 3rd floor girls bathroom. Repair or replace loose faucets in each bathroom.

Backflow can contaminate entire water supply with sewage or chemicals.

Plumbing must be properly installed with backflow prevention.

Air gaps at fixtures; Backflow preventers on connections; No cross-connections; Annual testing of devices; Proper drainage.

PLUMBING INSTALLED; PROPER BACKFLOW DEVICES - Comments: 5-205.15--Observed low water pressure for hot water in student's bathroom in 002 Classroom.

Backflow can contaminate entire water supply with sewage or chemicals.

Plumbing must be properly installed with backflow prevention.

Air gaps at fixtures; Backflow preventers on connections; No cross-connections; Annual testing of devices; Proper drainage.

PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: 6-201.11--Observed stained ceiling tiles from leaks in ceiling. Determine source of leak, and repair or replace all affected ceiling tiles.

Deteriorated facilities harbor pests and prevent proper cleaning.

Building must be maintained in good condition.

No holes in walls/ceilings; Doors and windows intact; Fixtures functional; No water damage; Surfaces intact.

PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: 6-501.114--Observed unnecessary items stored on the floor near dry storage area in break room and on the floor in the exterior 001 and 002 storage classes. Remove any unused items and store at least 6 inches from floor.

On ceilings and walls in the 2nd floor girls bathroom, 2nd floor boys bathroom, 3rd floor girls bathroom. Walls of dry storage area on first floor Repair all surfaces to be smooth and easily cleanable.
