FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED - Comments: 4-101.11(B, C, D, E) WATER CONDENSATION ON THE OUTSIDE, BETWEEN TWO FREEZER DOORS. IT APPEARS THAT THE RUBBER GASKET IS NOT FITTING PORPERLY. INSTRUCTED TO REPAIR AND MAINTAIN.

PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: 6-201.13 OBSERVED ONE BROKEN AND SEVERAL NOT TIGHT FITTING CEILING TILES IN THE BACK STORAGE AREA THAT MIGHT ALLOW PEST ENTRANCE. ALSO, OBSERVE THE PEALING OF ONE CEILING TILE IN THE KITCHEN. MUST REPAIR OR REPLACE CEILING TILES AND MAINTAIN.
